Tadalafil (Cialis): treating male Erectile Dysfunction
Tadalafil has the following functions and uses:
1. Treatment of erectile dysfunction (ED)
Mechanism for action:
-Tadalafil inhibits the type 5 phosphodiesterase enzyme (PDE-5), which is responsible for degrading nitrogen monoxide (no)-dependent on cyclic monophosphate guanosin (cGMP) in penile cavernous tissue. By inhibiting PDE-5, the concentration of cGMP increases, resulting in relaxation of smooth muscles in erectile organs and increased blood flow to them, facilitating erection.
Effectiveness:
-it has been shown to be effective in numerous clinical studies to improve the ability of men with SD to achieve and maintain an erection suitable for sexual activity.
2. Treatment of pulmonary arterial hypertension (HTPA)
Mechanism in HTPA:
-in HTPA, PDE-5 is overexpressed in the pulmonary vessels. By inhibiting this enzyme, tadalafil promotes relaxation of smooth vascular muscles in pulmonary vessels, reducing pulmonary vascular resistance, improving blood flow to the lungs and relieving pulmonary arterial pressure.
Clinical benefits:
-it can improve the exercise capacity, cardiac function and quality of life of patients with HTPA.
3. In some cases, for the treatment of erectile dysfunction secondary to certain medical diseases or treatments
Examples:
-in patients with diabetes mellitus, who often have a higher incidence of SD due to vascular and nervous changes associated with the disease. Tadalafil can help overcome erection problems in these patients.
-also in those who have suffered ovarian nerve damage after pelvic surgery, radiotherapy, or other medical interventions that may affect erectile function.
It is important to note that tadalafil should be prescribed by a doctor and consideration should be given to possible interactions with other medicinal products and contraindications, such as concomitant administration with nitrates, which can lead to a dangerous drop in blood pressure.
